New Student Registration Information

If you are a new student, please contact TeriLynn Porritt, the registrar, at (435) 257-2560 to begin the enrollment process.

You will need to bring the following with you:
  • Birth Certificate - Copy of full sized, state or country issued birth certifiate.
  • Immunization record - if you have questions about the current requirements for the State of Utah, please visit www.immunize-utah.org
    • Beginning with the 2015-16 School Year, all entering 7th Grade Students must present written proof of the following immunizations:
      • Tdap
      • 2 Measles, Mumps, Rubella (MMR)
      • 3 Hepatitis B
      • 2 Varicella (chickenpox)...or history of disease is acceptable.  
        • A parent must sign the verification statement on the school immunizatio record
      • 1 Meningococcal
      • 4 Polio -3 doses if 3rd dose was given on/after the 4th birthday
      • 2 Hepatitis A
      • 5 DTaP/DT - 4 doses of DTap if 4th dose was given on/after the 4th birthday; 3 doses of Td if starting series after age 7 with a single dose of Tdap preferred as the first dose.
  • Check out form from previous school, with current grades.
  • Special Education Record or 504 Plan  (if applicable)
  • To complete the registration pro process, student fee payment must be made.
    • Registration Fee Information 
    • Pro-rated fees are charged when enrolling your student
      • If you are eligible for a fee waiver, please fill out the "Fee Waiver Application and attach needed documents and our administration will review your file for approval/denial.
      • If you have a Horizon Card, we will need the 6 or 8 digit case number associated with that card.  You do not need any other verifcation of income if you have a Horizon Card.  Please bring your Horizon Card with you and we will make a copy of it to attach to your student's file.
  • Lunch - We request the payment of at least 3 days, $5.55, for your child's lunch balance at the time of registration.  If you are applying for free/reduced lunch, please complete the application .  Our district food service personnel will review your application for approval/denial.  You will be notified of their decision.  
    • One lunch card per year is provided for your student.
  • Additional fees that may be charged include:
    • $5.00 for PE Shirt (suggested white shirt, can be brought from home.)
    • $10.00 for PE Shorts (suggested red basketball shorts, can be brought from home.)
    • $35.00 per trimester instrument rental (you can provide your own intrument or rent from somewhere else.)
